The questionnaire can be filled out by each participant at www.dgcustomerfirst.com or at a department store that has been accepted. You won’t be able to get the prizes until you answer all of the survey questions.

To enter the contest, go to www.Dgcustomerfirst.com and use the survey code that you can find on your Dollar General receipt. Then, fill out the survey with your name, home, phone number, and email address.
- Entry By Mail
This is where you should send your card, which should be 3 1/2 inches by 5 inches and say “Mail-In Entry,” Dollar General Survey, PO Box 251328, West Bloomfield, Michigan 48325.
Include your full name (no initials), email address, street address (no PO Boxes), city, state, ZIP code, phone number, and birth date in the format MM/DD/YYYY.
